How to extend my U.S. stay?

I am inside the US with a B1/B2 visa and my maximum stay time is about to expire, 

how can I request an extension to stay longer time inside the U.S.?

For how long can a person usually stay inside the U.S. with a B1/B2 visa?

How do I know until what date I am allowed to stay in the US.? What is the form I-94?
